We are thereby delivering a system of interdependent interacting resources to foster better health and care, which will reduce costs,” states George Reigeluth, the President and CEO of Prosumer Health.
In conversation with CIO Applications magazine, Reigeluth mentions how their IHP is changing the face of modern care delivery by combining advances in healthcare information technology, AI, learning systems, remote patient monitoring, the data, learning, decision and cognitive sciences and behavioral psychology/economics into a single system centered around one patient at a time.
What are the existing challenges in the healthcare industry, and how has Covid exacerbated the situation?
The US healthcare delivery non-system has always been highly uncoordinated and disconnected, error prone with out-of-control costs and few standards for the quality of the data and analytics that go into care decisions. As a result, patients find it difficult to access and navigate their care path and to improve their health outcomes. These inefficiencies are evident from the rapid increase in clinician burnout, causing many caregivers to leave the profession altogether leading to a growing shortage of clinicians which further reduced patient access to care and their health outcomes, especially for those already underserved families and those suffering from chronic illnesses or deliver.

In response to COVID, we at Prosumer Health included a COVID-19 module within our IHP system that could help with early surveillance and risk stratification for COVID in addition to improving early detection and better management of chronic problems and acute issues. For instance, if someone was suffering from a sore throat, they could access their IHP from their phones and click on ‘sore throat,’ which is one of the symptoms of COVID-19. The person would then answer refinement questions about their sore throat from the most recent peer reviewed literature. The AIguided analytics engine assesses these answers in the context of the rest of the patient’s data in their Problem Oriented Health Record (POHR)and in the context of the relevant peer reviewed literature to produce a COVID risk stratification score for that patient, indicating whether they are highly likely, less likely, or not likely to be infected. If they are highly likely, the patient can either get tested right away or if they are less likely they can set up an intelligent telehealth call with their health coach and the PCP who receive all of the data and supporting documentation and risk stratification score before the call saving everyone time during these trying times.
Could you please elaborate on the IHPs and its functionalities?
Building on advances in several domains, including HIT, AI/MLS, RPM, telehealth, the data, learning, decision and cognitive sciences, and behavioural psychology, the patient’s experience with their IHP starts with meeting their health coach, who helps them bring together as much of their health and care data as possible into a Problem Oriented Health Record that is easy for the patient, PCP, health coach and others on the patient’s health team to read and understand. This data includes data on the patient’s Social Determinants of Health (SDoH), which determine about 80 percent of a person’s health status. The primary purpose of the health coach is to provide ongoing, dayto-day motivation, support, compassion and help to an individual with their acute and chronic health issues starting with helping the patient put together their health goals and care plan from the data in their POHR which is vetted by the AI analytics engine and the patient’s PCP. The other main job of the health coach is to help the patient adhere to their care plan and achieve their health goals through day-to-day reminders, nudges and support. This single integrated platform facilitates seamless collaboration and communication between the patients, health coaches, and healthcare providers via the shared data in the patient’s POHR, the AI analytics engine, the health/ care knowledge repository, the remote patient monitoring devices, the in-app texting capability and the communitybased social service organizations that the health coach enlists to help the patient address their SDoH, such as lack of transportation, etc. Rather than rely on population-based data, the PH IHP uses each individual’s data to provide greater accuracy, precision and thus efficacy to the myriad decisions that patients, their health coaches and clinicians make every day about someone’s health and care. Each person’s IHP has all the key drivers of a person’s health on each IHP. These drives include daily health measures from RPM devices for better early detection, prevention and management of chronic diseases, automated medication management system for better medication adherence, early detection and better management of acute issues, and nutrition and fitness activities for better health. With these integrated set of resources, each person is able to quickly, conveniently and proactively manage the key aspects of their health and care on a daily basis. Better 24/7 ongoing digital and human support and communication lead to greater patient participation, and having all the key drivers on one IHP and having greater efficacy of decision- making leads to better care, health and cost outcomes. With their own individualized health care delivery system or IHP, each person will be able to find their own path to better care and health – all of which is leading to a new collaborative, connected, continuous primary care model carried out with a new health team that will be centred around each patient and their IHP and health coach, which is taking many of the data collection, refinement, vetting, analysis, guidance option and documentation preparation tasks off the hands of clinicians, thus reducing their burden, enabling them to practice at the top of their license and to establish a new more positive relationship with their patients.
